Client Referral from 3rd party - Mental Health

This person would like to refer a new client to our agency.  Only use if a 3rd party is referring  client, such as the court, another agency, another program ect.  

1. Open up the TSGL Client Referral Form  
2. Gather the client information and complete form.  

Submit and give Closing.

Thank the caller for the information and you will get their  information over to our Intake coordinator who will follow up with them to set up an appointment. 

Create Ticket under  3P MH Referral for level 2     

Completed.
