New - Home Care Client / Form

This person is seek new home care services with our agency.

1. Logon to Axis Care, under the Marketing Tab at the top of screen and click Leads, Click Create Lead on right side of screen.
2. Gather the client information and complete the Personal Information and Residential Address sections. 
3. Ask the following questions and note responses in system  under the Lead Notes section: 

• Are you currently receiving home care services or have ever received home care services?    (Note answer)

If Yes: ask how many hours and what agency, if known. 

If No: say, not a problem, we will need your doctor to complete a form. 

- Who is your primary care physician?

- When is your next appointment?

We can send form directly to doctor (attached) or we can send it (Email/Fax) to you to have them complete.

• What type of insurance do you have (Only accepting Medicade) 

If No Medicade, would you be a private pay client? 

Save and give Closing.

Thank the caller for the information and you will get their  information over to the Intake coordinator who will follow up with you to schedule a visit to get services started with us. 

If new client, research/ call doctor's office get fax or email and send attached form over with Client name and DOB filled in at top.
